We’re proud to share that Joining the Dots, part of our Adult Wellbeing Services delivered in partnership with County Durham and Darlington NHS Foundation Trust (CDDFT), has been shortlisted for the Military and Civilian Health Partnership Award at the 2025 HSJ Awards.

 

This recognition comes as part of the Supporting Veterans to Access Cancer Information project, an inspiring initiative that brings together teams across CDDFT to improve access to vital health information for veterans.

 

Vicki Moffat, Programme Manager for Adult Wellbeing Services, played a key role in shaping and leading this work. Her dedication and leadership have been instrumental in driving forward meaningful, person-centred change.

 

This achievement highlights the power of collaboration and our shared commitment to improving lives through accessible, compassionate support.
