Formerly known as the Stockton Service Navigation Project (SSNP), the Pioneering Care Partnership (PCP) is pleased to announce a brand name change to the local service.
Now named Community Connect Stockton-on-Tees, the service's aim remains the same by helping people to reintegrate with society. Local people will have the opportunity to meet new people, attend hobby and social groups, including walking groups and health and wellbeing sessions, improve digital skills and make positive lifestyle changes.
Community Connect Stockton-on-Tees is available to those age 16+ in Stockton-on-Tees to offer one-to-one or group sessions to help you access local groups, activities and services in the community.
PCP is proud to delivery Community Connect Stockton-on-Tees to local communities, to strengthen its mission of 'Health, Wellbeing and Learning for All'.
