The PCP was originally established as a registered charity and company limited by guarantee in 1998.
The organisation was initially set up to support local people who may have become disadvantaged through caring responsibilities, age or ill health in Sedgefield Borough. However, the charity now aims to support people from across County Durham and Darlington to improve their health and wellbeing. Over the years people from across the North East have travelled to use some of the charities specialist services.
The PCP's mission is to ‘improve people’s health, wellbeing and quality of life, whilst reducing health inequalities across County Durham and Darlington.’

The PCP has a board of directors and trustees, who are responsible for the overall running of the organisation. The board is made up of local people, the majority of who are disabled people, people with ill health or carers.
